GAP Program Application

Please download and complete the application form below along with the following information:

1. Proof of Income: This could include recent pay stubs, proof of disability payments (such as an SSDI letter), or any other documentation showing your current income status.

2. Verification of Serious Mental Illness: We kindly request documentation verifying your serious mental illness. This could be in the form of a letter from your case manager, clinical provider, or any other relevant healthcare professional.

3. Proof of Sustainable Income: Please provide documentation demonstrating that you have sustainable income to maintain your housing after receiving assistance from the GAP program. This could include proof of employment, disability payments, or any other source of income.

Providing these documents will help us ensure that your application is processed accurately and efficiently. Once we receive the required documentation, we will continue with the review process.

 

Please note that the maximum amount of funds awarded is $1500.00, our turnaround time is two weeks, and you must have a consistent income with the ability to sustain housing if we assist.
